Sending sample to MiraVista from Canada

My vet has sent samples for Will but I am sending one from me today and not sure the best way.  My Dr gave me the little pill container thingy and filled in the requisition but not sure how to ship it.  Waiting for a reply from the vet but one of you might be able to help me faster.  Want to get the first mail out today.
