Help! Meds reducing potency?
« on: September 25, 2010, 07:31:18 PM »

Hi there, my vet (who has never treated blasto before) prescribed Prednisone and Pepcid with the ith. antifungal. We give Parker the antifungal 2x daily at least 2 hours away from the other meds. Should we not give her the Pepcid and/or Prednisone? We still aren't sure she has Blasto, but I certainly don't want to lose precious time dosing her up if what I am giving her is essentially crossing off the effectiveness of the antifungal. We will be testing her for Blasto this coming week. My original post is the Lab X one. Seeing a vet here who has treated Blasto is not a possibility - there are no documented cases in Newfoundland. Our weather doesn't get that hot and humid so I guess the spores don't live here. We think Parker picked this up in the Ottawa area - where we were living until August of this year.

Thank you,
Nicole

Hi Nicole -
Prednisone *can* help, but it also weakens the immune system.  We've found that it should be used with care and only sparingly to help them with the hardest breathing problems.
If you're giving the antifungal orally, antacids are contraindicated.  The antifungal will make them nauseous, but the antacids will decrease or even block absorption of the only thing that will save their lives.
Be sure to check the package inserts and research whichever antifungal you're using.  Also - research fungal pneumonias; you'll see immediately that prednisone is also contraindicated.  The immune system and the antifungal are your only weapons, make sure they're being optimized. A vet who hasn't done their homework with Blasto can be a liability: if you have to tell them their business, make sure you've done the research and come in locked and loaded.  It will help them to save lives down the line.

Thank you for replying. I can't blame my vet for this - she is just doing her best. We don't live in a Blasto area at all, Newfoundland has a cool climate so I don't think the fungus could spread/live here. She is partnering with me on this. If I tell her what I am discovering about the contraindications, she will agree with how we want to proceed.

We will not be administering Prednisone or Pepcid. I guess I should ready myself for bad days ahead if the antifungal starts working in time. I even see now that Parker has lesions like those of KoKo from "Beating Blasto".

This is so upsetting. I thought her appetite was a good sign when in fact it is likely because her antifungal isn't working yet.

Thank you for listening. Sorry for wailing.

Nicole -
Don't be sorry, and do please wail.  That's what this place is for.  Everyplace  And those lesions should provide your positive diagnosis.  Have your vet swab one and check it out. She'll see the blasto right there.
Here's what it looks like: http://botit.botany.wisc.edu/toms_fungi/apr2007.html

Everywhere is a blasto area, never doubt it.  Humans truck it everywhere in landscaping projects, construction, dirt, mulch etc. Spring and fall are the prime seasons for infection.
There are only places that are more inhospitable to it than others.  It's less likely to be found out west where there are fewer trees, less dirt, water and leaves, but one of our most helpful cases was Gunner from Texas.  Her owner was told that there's no such thing as Blasto in Texas.  The vet was wrong. She most likely got it from bad mulch, as my Dirtbike did.
Dirtbike was in the same shape as your Parker when diagnosed by a slide taken from a lesion. She's now 4 years old, and no sign of illness in the last 2 and 1/2 years.  It was hard, but she pulled through.
Gunner fought the good fight and beat it after a year, but by that time it had devastated her kidneys, and she passed.  But her story helped us saved so many others.  So stick with us, and even bring your vet here if she's serious about this.  I think she'll find it interesting and helpful.

Best-

Nicole...How is Parker doing?  Have you gotten the results yet?  We just got our results this morning...strong Positive.  Chance has been feeling much more energetic and happier since being on the meds, but the cough is sounding bad...deeper.  Just gotta give them all our love to help keep them strong!  Hang in there!!!
